A Clinical Study of Ischiorectal Abscess and Fistula



Abstract
This is a report of the studies of the 53 patients with the diagnosis of ischiorectal fistula and abscess, who operted at the Department of Surgery, Dong San Sacred Heart Hospital from January 1986 to February 1991.
@ES The result were as follows ;
@EN 1) The anal fistula comprised 25.1% of the cases of anorectal disease, excluding congenital anormaly and anorectal malignancy, 8.1% of which were the anorectal abscesses. The ischiorectal type of the anal fistula was 24.3%, and the
ischiorectal
abscess of the anorectal abscess was 17.5%.
2) It was more common in male with the male to female ratio of 5.62 : 1, and the peak incidence was in the 4th decades.
3) The most common cause of the ischiorectal fistula was postoperative recurrence of the previous operations for anorectal fistulous diseases, which was 41.5% of the cases, and 26.4% of the history of I & D for perianal abscess.
4) The duration of the symptom prior to admission was most common within a year with 41.5%, and more than 10 years with 9.4%.
5) The most common subjective symptom was pus discharge with an incidence of 75%.
6) Among the chest X-ray findings, 5.6% showed pulmonary tuberculosis lesion, and 3.6% was found to be active.
7) By the classification of Yukio Sumikoshi, 53.5% of the ischiorectal fistula was unilateral ischiorectal type, which was the most common type.
8) The internal opening was identified in 94.3% during operation, and the most common site of internal opening was posterior area with lithotomy position, which was 90.5%.
9) The mean duration of hospitalization was 12 days.
10) The period required for wound healing was less than 4 weeks in 39.6%, and more than 8 weeks in 9.4%.
11) Excluding the ischiorectal abscess, Modified Hanley's operation with muscle transposition was performed in 43 cases(81.1%).
12) The post-operative complications were recurrence(22.6%), delayed wound healing(3.8%), and transient partial fecal incontinence(1.9%).
13) The duration of recurrence after operation was most common within a year with 91.7%, and 3 cases relapsed more than twice.
